80578 (2024). Dr Tracey McLellan to the Associate Minister of Health: What steps has she taken, if any, to enable Home and Community Support Services (HCSS) to keep people well in their communities for longer and in reducing demand on Aged Residential Care and emergency and specialist services, as outlined in Recommendation 15 in the Aged Care Commissioner's March 2024 report?

Hon Casey Costello: Home and Community Support is a key input in supported early discharge models of care which support transition from hospital to home. The use of a restorative model of care emphasises individual goal setting to maintain independence as long as possible. I also refer the Member to my response to Written Parliamentary Question 80577 (2024).
