73708 (2024). Hon Dr Ayesha Verrall to the Minister of Health: What progress, if any, has occurred to address the recommendation that “Te Whatu Ora Southern review the circumstances of those patients identified as having been harmed in the harm registers referred to in this report, to ensure that ACC treatment injury claims have been made as appropriate”, as contained in the Health and Disability Commissioner’s report on Te Whatu Ora Southern’s non-surgical cancer service, as at 12 November 2024?
Hon Dr Shane Reti: I am advised that a report was completed on 24 May 2024 in response to an action identified following the publication of the HDC report. Work undertaken included a retrospective clinical review of all patients listed on the harm register, with ACC claims lodged if it was believed the threshold for treatment injury had been met. All instances of identified harm have been recorded in Safety1st and investigations commensurate with the actualised SAC rating have been completed.
