Hon Dr Ayesha Verrall to the Minister of Health: What actions, if any, have been taken in the quarter (April to June 2024) to implement the recommendation ‘Development of Pacific equity, community and family focussed models for care navigation should be investigated (community out into hospitals)’ as part of Te Whatu Ora Planned Care Taskforce Reset and Restore Plan?

Hon Dr Shane Reti: I am advised that during April to June 2024 quarter, Health New Zealand was able to provide care navigation for Pacific people accessing healthcare services. Health New Zealand’s national and regional teams are also working with primary and community providers to ensure Pacific people have support with First Specialist Appointments and have continuity of care when they are discharged from hospital and require community care. I am further advised that at regional level, Health NZ is working with Pacific providers to build stronger links with primary and community care providers to support access to welfare and social care supports.
