Hon Mark Mitchell: I am advised that of the incidents classified as a serious assault in double-bunked cells at Mt Eden Corrections Facility since 1 September 2024, there were nine instances where a Shared Accommodation Cell Risk Assessment was completed for both prisoners before they were placed together.
