57982 (2025). Dr Tracey McLellan to the Minister of Corrections: What advice, if any, has the Minister received on common risk factors identified by Corrections or the Office of the Inspectorate across the three recent deaths in double-bunked cells at Mt Eden Corrections Facility that are the subject of homicide investigations?
Hon Mark Mitchell: I am advised that the member’s question has been interpreted to relate to the deaths in double-bunked cells at Mt Eden Corrections Facility in September 2024, June 2025 and November 2025. While I have not received specific advice on common risk factors identified by Corrections or the Office of the Inspectorate across the three recent deaths in double-bunked cells at MECF that are the subject of homicide investigations, I did receive a verbal update from Corrections officials on a number of matters relating to the Shared Accommodation Cell Risk Assessment (SACRA) and double bunking on 16 December 2025. This included: • details of the three recent deaths at MECF and the circumstances around each incident • Corrections’ current response to the incidents including an overview of work underway to review the SACRA, and the implementation of a number of immediate changes to guidance and the application of SACRA at prison sites while the review is completed. This is also my response to WPQ 58661 (2025).
