43706 (2024). Ingrid Leary to the Minister for Mental Health: Does the Minister agree with Coroner Alison Mills who recommended a review of discharge planning, ongoing education for staff on family engagement with regular audits of the current policy, and implementing systemic reviews for all serious event reporting, with reference to the death of a Waikato mental health patient; if so, what, if anything, will he do to address this?

Hon Matt Doocey: I am advised that whenever there is a serious or unexpected event, Health New Zealand has a process of review to identify any changes which can be made to improve patient safety and care.
