36124 (2026). Hon Willow-Jean Prime to the Minister for Children: Further to 34057 (2026), at what time and on what date was the Serious Event Notification made following the alleged incident at Oranga Tamariki's Korowai Manaaki?

Hon Karen Chhour: Subsequent to my answer to WPQ 34057 (2026), I have been advised by Oranga Tamariki that a Serious Event Notification (SEN) was not completed in this instance, however it was escalated to senior leadership through an incident briefing process. I am further advised that a SEN is a notification tool, but it is not always mandatory to use and is not the only mechanism available to notify/escalate incidents, in particular where the matter may be more historic or there is no immediate risk of harm identified. In this case, I am advised the residence and the National Support Services team used an incident briefing process, which provided largely the same information as a SEN would.
