30844 (2025). Ingrid Leary to the Minister for Mental Health: Have any children or adolescent deaths, from suicide, been referred to a coroner, after being turned away from treatment with Child, Adolescent and Family Mental Health Services, if any?

Hon Matt Doocey: This information is held by the Office of the Chief Coroner, at the Ministry of Justice. However, I am advised that all suspected self-inflicted deaths are reviewed by a coroner who can investigate and issue recommendations to services where appropriate.
