27088 (2026). Hon Dr Ayesha Verrall to the Minister of Health: How many serious adverse events were reported by each Health New Zealand district in each calendar month since 1 July 2022, broken down by district and by event category as used in Health New Zealand's own reporting?

Hon Simeon Brown: I am advised that a total of 3,833 serious adverse events were reported between 1 July 2022 and 31 May 2026. Of these, 539 were assigned a Severity Assessment Code 1 rating, and 3,294 were assigned a Severity Assessment Code 2 rating. I refer the Member to the attached tables which show this information broken down by district, calendar month and event category rating within each of the financial years.
