13987 (2026). Tamatha Paul to the Minister of Police: What specific IPCA recommendations arising from deaths in Police custody have been fully implemented to date, if any, and which remain outstanding?

Hon Mark Mitchell: I am advised that there are 10 recommendations issued by the Independent Police Conduct Authority (IPCA) arising from deaths in Police custody that have been formally recorded and tracked in the Recommendations Database. Of these, seven recommendations are recorded as completed, confirming that Police have implemented the actions agreed in response to each of those individual recommendations. These include recommendations arising from the following IPCA reports: • Police fail in their Duty of Care to Allen Ball in Hawera (two recommendations completed) • Concern that Police Failed to Discharge Their Duty of Care to a Man in Custody (subsequent Police action completed) • IPCA investigation into the death of Alo Ngata following his arrest in Auckland (completed and formally closed); and • Death of Mr Z in Police custody in Auckland (three recommendations completed). Three recommendations remain outstanding and are recorded as in progress. These are: • Two recommendations arising from the IPCA report Woman Dies in Police Custody at Gisborne, both of which are marked as “Started – On Track”; and • One recommendation from the IPCA report Police Fail in their Duty of Care to Allen Ball in Hawera remains open and marked as “Started – On Track”.
