Hon Dr Ayesha Verrall to the Minister of Health: What progress, if any, has been made in the quarter October – December 2023 to ‘Prototype admission avoidance, early discharge and home-based care, including remote monitoring pilots; and refocus community nursing, allied health and the Needs Assessment and Service Coordination services to be part of comprehensive primary and community care teams’ under section 3.3 ‘Stronger primary and community care’ within Te Whatu Ora’s Te Pae Tata: Interim New Zealand Health Plan 2022?

Hon Dr Shane Reti: I am advised that during the October - December 2023 quarter, local tailoring and design of the Comprehensive Primary and Community Care Teams (CPCT) model was underway in the regions. I am further advised that contracting relating to this work was also progressed in the quarter.
