Dr Tracey McLellan to the Minister of Corrections: What monitoring, if any, has been undertaken to ensure that the changes made following the review by the Office of the Inspectorate of the management of Malachi Subecz’s mother by the Department of Corrections is being applied consistently across all prisons and probation sites?

Hon Mark Mitchell: I am advised that as part of their business-as-usual operations, Corrections are constantly reviewing staff practice across prisons and community corrections sites to support continuous practice improvement and ensure that processes are consistently applied.
